Scrubs are a good thing to use, especially in the winter, when your skin tends to get dry and flaky.
Lip scrubs are similar to face scrubs in their components and uses. Both generally contain two main ingredients where one would act as a rich moisturizer (namely oils) and the other as an exfoliant.
Take a small amount on your fingers and gently apply the scrub in a circular motion all over damp lips for 2-3 minutes. Make sure you do not scrub too hard or use harsh ingredients which could result in wounding your skin.
Leave it on for at least 30 seconds before rinsing it off with warm water and pat drying your skin with a towel.
You should never stop caring for your skin so perhaps you could use a scrub at least once or maximum twice a week (in case you have oily skin). More than that may irritate your skin or make it excessively dry.

Having said that, you have to make sure not to overdo the exfoliation. I would suggest you try using a scrub once a week and watch your skin. If there is no visible irritation, continue with the same. If you have sensitive skin, try consulting your doctor before using anything like scrubs or various other skin products for exfoliation.
